Title Forms

Motor Vehicle Title Application (MV-1) This form is used to apply for a Georgia certificate of title and should be completed by the vehicle purchaser/owner.
Affidavit to Certify Immediate Family Relationship (MV-16) This affidavit is used to certify that a vehicle was transferred between immediate family members by gift, sale, purchase, or inheritance. Eligibility is limited to spouse, parent, child, sibling, grandparent, or grandchild.
Lien Release (T-4) This form is completed by the lienholder to release a lien/security interest on a vehicle title. All signatures must be original and notarized.
Limited Power of Attorney (T-8) This form authorizes another person to complete title and tag transactions on your behalf.
Affidavit of Correction (T-11) This form is used to explain and correct errors made in the assignment section of a title or other ownership document. It cannot be used if information has been completely scratched out, covered with correction fluid, or altered beyond a simple correction.
Affidavit of Repossession (T-16) This form is used by lienholders when repossessing a vehicle.
Affidavit of Inheritance (T-20) This form is used to transfer ownership from a deceased owner to an heir.
Affidavit of Fact for a Motorcycle or Scooter (T-228) This form certifies that a motorcycle or scooter meets Georgia roadworthiness requirements.
Odometer Discrepancy Affidavit (T-107) This form is used to explain mileage discrepancies or corrections.
Affidavit of Georgia Certificate of Title Lost in the Mail (T-216) This form is used when a Georgia title was not received by mail.

Registration Forms

Disabled Person’s Parking Affidavit (MV-9D) This form is used to apply for a disabled person’s parking placard or plate and must be completed by the applicant and a licensed medical provider, when required.
Bill of Sale (T-7) This form may be used to document the sale and transfer of vehicle ownership between buyer and seller.
Application for Replacement Plate and/or Decal (MV-7) This form is used to request a replacement license plate and/or decal if it has been lost, stolen, damaged, or not received. A police or incident report may be required.
Voluntary Cancellation (MV-18J) This form is used to voluntarily cancel a vehicle registration. Registrations should be canceled before terminating insurance coverage to avoid penalties. Requests may be completed in person, by mail, or online.
Temporary Plate Extension Application (T-226) This form is used to request a one-time extension of a temporary operating permit after having purchased a vehicle from a licensed Georgia dealer.
College/University Plate Application (MV-9C) This form is used to apply for a college or university specialty license plate.
Military Specialty Plate Application (MV-9W) This form is used to apply for qualifying military and veteran license plates.
Relinquish a Specialty or Personalized Plate (T-237) This form is used to surrender a specialty or personalized plate.
VIN Verification Form (T-22B) This form is used to verify the vehicle identification number (VIN) of a vehicle or trailer.
Motor Vehicle Data (MV-20) This form is used to request motor vehicle records or information.
Prestige License Plate Application (MV-9B) This form is used to apply for a prestige (personalized) license plate.
